1 - REGISTER OR DEACTIVATE A PRESCRIBER FOR SURESCRIPTS

 

REGISTER A PRESCRIBER

The Client should submit this request by creating a case in MICentral that includes the following information

  • NAME OF PRESCRIBER
  • DEA
  • NPI
  • STATE LICENSE NUMBER

The information provided should match what is displayed in the NPPES NPI Registry https://npiregistry.cms.hhs.gov/

The SureScripts registration cannot be completed, if the information is incorrect.

 

Once the information is received, it will then be forwarded to Customer Success.

An Addendum to register the Prescriber for SureScripts will be created and submitted to the Customer.

After the signed Addendum is returned, it will be sent to Accounting for approval.

Once approved, the information is sent back to Support and the Prescriber is then registered for SureScripts.

 

DEACTIVATE A PRESCRIBER

When a Prescriber leaves or will be leaving a practice, the Client should submit a case in MICentral to deactivate the SureScripts registration. Please provide the following information.

  • NAME OF PRESCRIBER
  • DEA
  • NPI
  • STATE LICENSE NUMBER
  • THE DATE OF THE DEACTIVATION

The information provided should match what is displayed in the NPPES NPI Registry https://npiregistry.cms.hhs.gov/

 

Once the information is received, it will then be forwarded to Customer Success.

An Addendum to deactivate the Prescriber from SureScripts will be created and submitted to the Customer.

After the signed Addendum is returned, it will be sent to Accounting for approval.

Once approved, the information is sent back to Support and the Prescriber is then deactivated from SureScripts.
